The definition of a questionnaire: A questionnaire  is a document that contains a range of structured questions, each with a set of possible responses, submitted to a statistically significant number of people to gain information for a survey. This document can be on paper or online. To get structured questions you need to use a precise wording. It is important that each respondent gets exactly the same range of questions. That’s the reason why questionnaire design is so important.
